REGISTRATION: By mail only, through lottery, unless slots remain the day of competition; then only as
available and with proper waiver form endorsements. This form must be completely filled in and postmarked
by April 17, 2009 (Eastern) or May 1, 2009 (Western) to be considered pre-registered. The field in
